Support Windows UNC pathnames

Description

#P"machine-name/share-name/path/goes/here/" does not work. It removes the initial slash, producing the path #P"/machine-name/share-name/path/goes/here/" which, of course, is incorrect.

This most likely involves the #P reader, and possibly also mapping Windows UNC path components to CL pathname components.

I imagine that PATHNAME-HOST would map to "machine-name" in the example, and that PATHNAME-DEVICE would map to "share-name", and the remainder of the path would be as usual.
